Patient Forms

Upon entering our office our receptionist will greet you and welcome you as a member of our family. We will then request you fill out our new patient forms. This paperwork provides us with general information about yourself and your condition. You can also download our forms from the "online forms" section of this website. Having the necessary paperwork completed before you arrive at the office will shorten the amount of time necessary for your first visit.


Consultation

Next you will have a consultation with Dr. Mike or Dr. Matt to discuss your health-related problems, concerns, as well as potential treatment options. Your first visit is designed for Dr. Mike and Dr. Matt to learn more about you, your condition, and your expectations in order to determine how our office and staff can help you meet your goals. Our doctors will take the time to really listen to "you" and address all of your questions and concerns.


Examination

After your consultation, Dr. Mike or Dr. Matt will perform a complete chiropractic, neurological, orthopedic and functional examination. This comprehensive examination tests your reflexes, your ability to turn and bend as well as other standard neurological, orthopedic, postural, and physical examinations. It will also give our doctors insight into your ability to perform many of your everyday activities of daily living (such as bending down or reaching behind your back) in the most efficient manner. All of this information will help our doctors tailor a specific treatment protocol that will best serve each patients' specific needs. Nothing will be done in our office without your consent and our doctors will discuss with you each and every treatment option.


X-Ray Studies

Your specific condition may require us to take x-rays to get a full evaluation of a patient. X-rays help us rule out more serious conditions or assist us in developing the most effective treatment plan for you.  Most people are amazed once they see their x-rays and can often immediately identify their misalignments and degeneration themselves.


Report of Findings

Once all the information has been gathered and examinations have been performed, Dr. Mike and Dr. Matt will give you a report of findings to answer the four most popular questions: 

  1. Can you help me?
  2. What do you recommend that I do to get better?
  3. How much is it going to cost?
  4. How long will it take?

After discussing with you your history and your goals, examining your spine and reviewing any x-rays or tests, Dr. Mike and Dr. Matt will discuss recommendations with you. If you have a condition that requires care with other providers, the doctor will tell you. If Dr. Mike and Dr. Matt believe we can help you, we will recommend treatment and a wellness program.


Treatment

Patients will generally be provided with their first treatment at the conclusion of the first appointment. This may include spinal adjustments, physical therapies, and/or soft tissue therapies. Treatment is interactive with you so you can express any concerns you have about the style of adjustment you will be most comfortable with.


Wellness Program

Prior to leaving, patients will be given a wellness program that can be conducted at home. If you are in pain when you first come into our office this may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises, water intake requirements and/or stretches.  If you desire an overall wellness lifestyle, our wellness team will work with you to create good habits and routines that support your success.  Every patient is unique and therefore requires a unique and customized wellness plan.  The foundation of our overall wellness lifestyle is maintaining healthy posture and good spinal alignment, eating a healthy diet, taking supplements, keeping your weight under control and having a method for stress relief.
